Custom form collecting information after checkout by Kamran Jamshidi

Hi there,

I'm looking for quotes for someone to help me develop a custom form that pops-up after someone has gone through my checkout.

The idea is that after a customer has bought the product, they are given the option to provide names, emails and workplace addresses of up to 5 of their friends, to whom we will send a free sample on their behalf.

We then want the information from that form to be sent to our email.
