Small Newsletter Opt-In Tweaks to Duke Theme (by Clean Themes) by Darnell Brown

Hello Great Developers of the World,

I'm in need of some changes to my Shopify theme with regards to my newsletter opt-in segment in the footer of my website. I have successfully linked an embedded form from my MailChimp account to my website's newsletter. In the Duke theme (by Clean Themes), the opt-in form is programmed to only include the required field for an email address. However, I now need that segment to include required fields for First Name and Last Name that precede the Email Address field; all 3 fields should be in a single row. I've tried this already on my own, but when I copy/paste the full embedded code from MailChimp's end, of course, the styling of the Shopify store theme is not carried over, thus the opt-in form is completely out of sync from an aesthetic standpoint.

Primarily, I need help with embedding the full MailChimp newsletter form code so that it's styled to match the look of my site and so that all 3 fields are on a single line, which ideally is rather simple for a developer to do. Secondarily, I would like to set up a super-simple newsletter opt-in page that is its own dedicated page that is a visual replica of my Login or Register page. There should be 3 fields, all stacked upon one another: First Name, Last Name, and Email Address. This form would link to my same newsletter, it's just an independent page that I can forward to potential customers. Can anyone help me?
